Prescribing Support Services is a leading provider of medicines management and clinical support to GP practices, commissioning groups and PCTs. 

We have an opportunity for a practice based pharmacist to support practices in the Bradford area.  The role requires the delivery of the prescribing and medicines management agenda at practice and individual patient level. The post holder will show skills in organization, communication and working with other health care professionals and ideally they will have a post graduate qualification and practice based experience.

Hours: 20 hours

Role

  1.  Review of all discharge advice medicines information (medicines reconciliation and a medication review)
  2. Help in medicines management aspects of QOF especially were there is large medicines component e.g. atrial fibrilliation , osteoporosis, CVD etc . This would involve identify patients who are suitable for treatment and seeing them to discuss treatment options.
  3. Medicines information query handling for GPs including dealing with patient queries.
  4. Assistance in pathway redesign where medicines a large component
  5. Management of patients who are at high risk of hospital readmission where there is polypharmacy and social risk e.g. case finding and management, domiciliary medication review, working alongside community matrons
  6. Drug budget and prescribing quality management e.g. formulary development, cost savings opportunities, KPPIs.
  7. Repeat prescribing management e.g. picking up the majority of reauthorization queries for repeats, working with community pharmacists and patients to reduce waste ordering,
  8. Managing dosette box patients to improve safety of prescribing
  9. Nursing home medication reviews and recommendations to GPs.
  10. Annual review of all scripts as above.
  11. Check that required pathology checks regards medication are done and check if on ICE system.
  12. Monitoring of DMARDS and similar medication quality & safety audits. 
  13. Liaising with Scripts team as first port of call for queries.  Assist with training and development of scripts team staff where appropriate.
  14. Attend practice meetings and other relevant meetings in the healthcare district on behalf of the practice.
  15. Any other prescribing related duties.
